We are seeking a qualified and empathetic Social Worker to join our multidisciplinary team. Responsibilities include..
Conduct comprehensive assessments to understand client needs, strengths, and challenges.
Develop and implement individualised care plans in collaboration with clients and their families.
Provide counselling and emotional support to clients dealing with complex social and personal issues.
Facilitate access to community resources, support services, and advocacy networks.
Collaborate with other health professionals to ensure integrated and effective care.
Maintain accurate and up-to-date client records in compliance with organisational and regulatory standards.
Participate in team meetings, supervision, and professional development opportunities.
Essential Qualifications and Skills:
Bachelor’s or Master’s degree in Social Work.
Demonstrated experience in social work practice, preferably in a healthcare or community setting.
Strong knowledge of social work principles, ethics, and frameworks.
Excellent communication and interpersonal skills.
Ability to manage a diverse caseload and prioritise tasks effectively.
A current driver’s licence and Working with Children Check (or willingness to obtain).
